As of May 1, Capital Blue Cross and Heritage’s medical practices will share claims and medical records to boost patient outreach, particularly those with chronic conditions, the insurer and Heritage said. Capital Blue Cross is based in Susquehanna Township and Heritage is based in Lemoyne.

The partnership will help patients establish and manage a plan of care that should give them more time with doctors, improve patient health and reduce waste and care fragmentation, Capital Blue Cross said.

The partnership includes new payment methods and gives incentives for meeting benchmarks that produce high-quality care, customer service and health, the insurer said.

Heritage operates 19 medical offices.
